Attn: Vendor:
The Federal Tax Law requires us to have your Taxpayer Identification Number (TIN) on file. Under Federal Regulation, you are required to provide us with this information on the W-9 form included with this letter. If you fail to furnish us with this information within 30 days, we will be forced to begin backup withholding on all payments to you.
We will not have to file an annual Information Return, Form 1099 Misc. for you if you are a corporation, tax-exempt organization, government agency, or other exempt payee. However, the law requires that you give us your TIN number in addition to informing us of your payee type. If you do business as an individual or sole proprietor, your TIN number is your social security number; otherwise your Federal Employer Identification number serves as your TIN.
If your social security number is your TIN number, your name must appear first, with your business name being a DBA. (Example: SS# 000-00-0000 John Doe d.b.a. Johns Cleaning Service).
Please check your payee type, complete the enclosed W-9 form, and return both to the address listed above, Attn: REO Department, Floor 3B.
